Functional Home Transformations, LLC (FHT) is a mobile outpatient occupational therapy clinic, meaning that all treatment is completed in clients’ homes. The ultimate goal is to provide clients and their caregivers the skills, techniques, resources, and confidence to be as safe and independent in their homes as possible. FHT’s 55+ years old. They are active adults who want to be proactive and learn ways to age in place. They are also adults who may have physical or cognitive limitations that make every day tasks difficult, but are motivated maximize their abilities. Clients may be caregivers who need guidance on practices and techniques to provide the best care for someone while staying safe and healthy themselves. FHT also specializes in home modification consultations for the client’s specific needs, fall prevention interventions, and durable medical equipment evaluations.
